INSTITUTE OF ALLIED HEALTH SCIENCES   22nd March, 2017 ADVERTISEMENT INVITATION FOR ADMISSION INTO DIPLOMA IN MEDICAL LABORATORY SCIENCES, PHARMACEUTICAL SCIENCES AND DIAGNOSTIC RADIOGRAPHY PRORAMME FOR THE ACADEMIC YEAR 2017/18

The Catholic University of Health and Allied Sciences is pleased to invite applications from qualified candidates who completed either form IV or form VI or candidates with equivalent qualifications wishing to pursue studies in Diploma in Medical Laboratory Sciences, Pharmaceutical Sciences and Diagnostic Radiography at the Catholic University of Health and Allied Sciences for the academic year 2017/18.

1. PROGRAM: Diploma in Medical Laboratory Sciences, Pharmaceutical Sciences and Diagnostic Radiography.

2. DURATION: Three (3) years.

3. ENTRANCE QUALIFICATIONS:

 a) Direct entry: i) A holder of form VI certificate (‘A’ level) with one principal and two subsidiaries in the science subject of Biology, Chemistry, Physics or Mathematics.                          

   OR ii) A holder of form IV certificate (‘O’ lever) of Secondary school education with threee credits in Biology, Chemistry, Physics or Mathematics.

b) Equivalent qualification: Laboratory Assistant, Pharmaceutical Assistant and Radiographic Assistant from recognized institutions with at least two years expereience in Clinical laboratory Practice, Pharmacy Practice and Radiographic practice provided the applicant has two passes in science subjects in ordinary level secondary education.

(c)  Candidates with equivalent qualifications must sit and pass entrance examination in addition to the qualifications mentioned above.

4. INSTRUCTIONS TO APPLICANTS: a) Applicants must carefully read the information above before attempting to fill the application forms.

b) Applicants must give full particulars of citizenship (including a copy of the birth certificate) and enclosing photocopies of heir academic certificates.  Applicants with academic certificates other than those issued by the National Examinations Council of Tanzania (NECTA) must submit a translation by the NECTA indicating equivalent grades to Tanzania ones.  This is in addition to submission of the original grades.

c) Applicants must pay a non-refundable application fee of T.shs. thirty thousands only (30,000/=) into Catholic University College Account Number: 01J1054045500 CRDB (Bugando branch) and submit the original pay-in-slip together with the application forms.

d) Applicants must ascertain themselves that they possess the minimum entrance qualifications befor they pay fees as the fees will not be refunded under any circumstances.
